What is Carescribr?
Carescribr is a charting application that helps primary care teams gather patient information and organize it into a format that is useful for their EMR note and pre-visit plan. The app helps patients during their office visit by providing updated information about their journey to medical staff through an automated tracking board.
What type of information does the Carescribr tablet collect?
Carescribr was designed specifically to assist medical teams with documenting the patient’s visit. It does not ask for co-pay payments or perform insurance verification. The app exclusively focuses on displaying valuable, relevant and actionable information to medical teams about a patient’s symptoms, conditions, risk factors, validated questionnaires and preventive screenings. Some examples include cough, chest pain, back pain, asthma follow-up, hypertension follow-up, well child exams (at recommended intervals), annual physical exams, Pediatric Symptom Checklist (PSC), Modified Checklist for Autism in Toddlers, Revised (M-CHAT-R) , Patient Health Questionnaire (PHQ-9), General Anxiety Disorder Screener (GAD-7).
Is Carescribr secure and HIPAA compliant?
The Carescribr team takes security and patient privacy very seriously. A dedicated, top-notch security team protects the Carescribr application and its database. HIPAA compliance is ensured through constant monitoring of data use. We work with all customers via a Business Associate Agreement (BAA) to ensure shared security responsibility.
What do I get for my subscription?
Subscriptions include full access to all questionnaire content without limitations and are structured to maximize benefit to the medical team. The monthly subscription includes speech recognition capability and user accounts for 1 medical provider, 1 nurse, 1 medical assistant, 1 front desk staff member and 2 kiosks.

Can I add more medical providers?
Subscriptions are team-based with the medical provider (MD/DO, NP, PA) operating as the team leader. Clinics can add additional medical providers by creating another medical team and connecting staff to their provider’s team.
Can I add more kiosks and medical staff?
Yes! We fully understand that not every team is the same size or has the same schedule. We included 2 kiosk accounts as part of the team subscription since our experience and the available research has shown 2 kiosks to be the bare minimum. We recommend having 3 kiosks per medical provider if patients are scheduled for every 15-20 mins to avoid any bottlenecks in the waiting room.
Is Carescribr another EMR?
Carescribr is not an EMR. The application does create a portable personal health record for each patient that can be shared with any Carescribr-enabled clinic. It solves the workflow problems that your EMR does not address.
Does Carescribr integrate with my clinic’s EMR?
Currently Carescribr does not integrate with EMR vendors. When we started creating Carescribr many smaller clinics expressed concern about having the staffing or IT resources to manage an integration. For that reason we designed Carescribr so it provides significant value immediately, without having to worry about integration. That being said EMR integration is the future and part of the platform road map.
There are symptoms and medical conditions that I treat that the app doesn’t ask about yet. Can I add more questionnaires?
We would love to hear more about your practice and how we can expand Carescribr’s clinical scope. We are actively adding more symptoms, conditions and optional questionnaires all the time. Optional questions give providers flexibility to ask additional questions to fit their workflow. Please send your feedback inside the application or contact us via email.
I am not a primary care provider, can I still use Carescribr?
Carescribr is currently designed to function best in a primary care office, specifically for outpatient General Internal Medicine, Family Medicine and General Pediatrics. We will be expanded to all specialties and settings. Please contact us if you would like to be part of expanding the application into your specialty or practice.
What type of tablet can I use for the kiosk?
Carescribr requires iPad models (iPad, iPad Air, iPad Pro, iPad mini) compatible with iOS 11 or later. We recommend using the newest model to ensure best performance.
Do you plan to build the app for Android tablets?
We currently do not have plans to expand to Android or Windows tablets.
How do I prevent security issues and protect against tablet theft?
The Carescribr license agreement and business associate agreement requires physical and digital security measures to continue with a Carescribr subscription. We strongly recommend all iPad tablets be securely mounted inside a kiosk enclosure that prevents physical tampering and theft. Every medical team can remotely shut down a tablet’s access to the application in the case of tablet theft.
